What is Lutetium Therapy?
Lutetium therapy is a modern type of targeted cancer treatment that uses a small amount of radioactive material called Lutetium-177.
This radioactive substance is attached to a special molecule that can find and stick to certain cancer cells. Once it reaches these cells, it delivers radiation directly to them, damaging the cancer while leaving most healthy cells unharmed.
How does Lutetium therapy work?
Lutetium therapy combines advanced science with targeted accuracy, focusing directly on cancer cells while protecting healthy tissue. Each step is designed to deliver effective treatment with minimal discomfort, giving patients a clearer path toward recovery.
1. Preparation
Before treatment begins, doctors confirm that your cancer has the correct type of “targets” (receptors) for Lutetium Therapy. This is done using specialized imaging scans, such as a PET/CT, to ensure the treatment will be effective for you.
2. Injection of the medicine
Lutetium-177 is linked to a special “carrier” molecule designed to find cancer cells. This combination is administered through a small needle into a vein in your arm or hand (intravenous injection). The process is quick and painless, allowing the medicine to enter your bloodstream and circulate throughout your body to reach and target cancer cells.
3. Travelling through the body
Once in the bloodstream, the carrier molecule acts like a smart GPS, circulating throughout the body in search of cancer cells with the matching receptors.
4. Finding and binding
When the carrier detects these receptors, it locks onto the cancer cells, placing Lutetium-177 exactly where it needs to be, directly on the tumor cells.
5. Delivering the radiation
Lutetium-177 releases low-energy beta radiation over a short range. This precision ensures the radiation targets only the attached cancer cells while minimizing damage to surrounding healthy tissue.
6. Damaging the cancer cells
The beta radiation damages the DNA of cancer cells, preventing them from dividing and growing. Over time, these damaged cells die, and the tumor can shrink.
7. Natural clearance from the body
Any lutetium that does not attach to cancer cells is naturally filtered out, mainly through urine, over the next few days. Patients are encouraged to drink plenty of fluids to help this process.
8. Repeat sessions if needed
For optimal results, treatment may be given in several cycles spaced a few weeks apart. Between sessions, doctors monitor your progress using scans and blood tests.
After the treatment is administered, most patients stay in the hospital for a short observation period, often just a few hours, before going home the same day.
9. Treatment duration and safety precautions
The entire appointment, including preparation and monitoring, usually takes half a day. You can typically resume normal activities within 24–48 hours, although your doctor may recommend avoiding close contact with young children or pregnant women for a short period due to the small amount of radiation still in your body.

Who can receive Lutetium therapy?
Lutetium therapy is not suitable for every cancer patient; it is a highly targeted treatment designed for specific tumor types that have special “receptors” on their surface. These receptors act like docking stations that allow the Lutetium-177 medicine to attach and deliver its radiation directly to the cancer cells.
Cancers commonly treated
The therapy is most often used for:
Neuroendocrine tumors (NETs): especially those that produce hormones and have somatostatin receptors.
Metastatic castration-resistant prostate cancer (mCRPC): particularly tumors that show PSMA (prostate-specific membrane antigen) on their surface.
Receptor testing: the key step
Before treatment, your doctor will order a special PET-CT or SPECT-CT scan using a tracer that binds to these receptors (for example, Ga-68 DOTATATE for NETs or Ga-68 PSMA for prostate cancer).
- If the scan shows high uptake in the tumors, you are likely a candidate for Lutetium Therapy.
- If the receptors are absent or very low, the therapy will not be effective.
General eligibility criteria
Most patients considered for Lutetium Therapy:
- Have cancer that has spread or cannot be removed surgically.
- Have tried other standard treatments (such as surgery, hormone therapy, chemotherapy, or external beam radiotherapy),but the disease has either progressed or returned.
- Have enough overall health and organ function (especially kidneys and bone marrow) to tolerate the therapy.
- Do not have severe bone marrow suppression or kidney impairment.
Not suitable for
Lutetium therapy may not be recommended for:
- Pregnant or breastfeeding women.
- Patients with severe kidney or liver failure.
- Patients with extremely low blood cell counts.
Benefits of Lutetium Therapy
Lutetium therapy is a promising option for people with certain advanced cancers, offering targeted treatment with fewer side effects than many traditional approaches. Beyond its precision, this treatment brings a range of advantages that make it an important option for many patients:
- Delivering radiation directly to cancer cells while sparing most healthy tissue
- Treating advanced disease effectively, even when cancer has spread or resisted other treatments
- Causing minimal pain, as it is given through a simple intravenous injection without surgery
- Producing fewer side effects compared to chemotherapy or external beam radiotherapy
- Improving quality of life by relieving symptoms and slowing cancer growth
- Allowing repeat treatment in multiple cycles for better long-term results
Clinical trials have shown that Lutetium Therapy can improve survival rates and help control symptoms in eligible patients, providing a solid scientific foundation for its use
